Welcome to Mt. Ararat High School Athletic Registration

-WINTER 2020-2021-

Please carefully review the information below before registering for an activity.

 

There are three (3) things that must be thoroughly completed before a student will be permitted to participate in a Winter 2020-2021 sport:

1. Complete this online registration.

2. Meet all academic eligibility guidelines as outlined in the 2020-2021 Mt. Ararat High School Student Handbook

3. Successfully complete the ImPACT Baseline Concussion Test as directed by the Athletic Department. This will be administered in November/December, prior to the start of regular season competition, to all necessary registrants by the Athletic Trainer.

All Mt. Ararat High School students (grades 9-12) wishing to participate in a Winter Athletic program at the high school MUST complete this online registration BEFORE THE FIRST DAY OF ANY PRACTICE – CURRENTLY SCHEDULED FOR NOVEMBER 16, 2020.

Parents, please make sure that you and your student-athlete go through this entire online registration carefully and provide all necessary information. Also, be sure to read and review all materials and any supplemental documents referenced in this registration.

INFORMATION NEEDED TO REGISTER

It will be helpful to have the following information handy to complete the registration process:

  • A copy of your most recent physical evaluation documentation from your doctor

  • Doctor’s Contact Information - Name, Practice, Address, Phone

  • Health Insurance Information - Company, Group #, Policy #
    HEALTH INSURANCE COVERAGE IS REQUIRED FOR PARTICIPATION IN MT. ARARAT HIGH SCHOOL ATHLETIC PROGRAMS.  IF YOU DO NOT HAVE THIS COVERAGE, YOU WILL NEED TO PURCHASE IT ON YOUR OWN OR THROUGH THE SCHOOL.  INFORMATION ON SCHOOL-BASED INSURANCE CAN BE DOWNLOADED BY CLICKING HERE.

PHYSICAL EXAMS
Each Mt. Ararat High School student-athlete MUST provide written documentation of a physical examination by a health care professional within the past 24 months from the first day of practice/tryouts (November 16, 2020) and it must be valid until the end of the Winter sport season (February 27, 2021).

All written proof of physical examinations must be submitted to the Mt. Ararat High School Athletic office, either on paper or through this registration system. Please note on the documentation the specific Winter sport that your son/daughter will be participating in. Failure to submit updated physical exam documentation could result in a delay in the student-athlete's participation.

Sample documentation to take with you to your physician for their completion during the physical exam may be downloaded HERE.  

All physicals must be dated and signed by the physician and contain the date of the actual physical exam. The physical form must state that the student is cleared to participate in sports and activities. This online registration program will allow you to upload a copy of your most recent physical exam document.

Special Note for the 2020-2021 School Year:  If you are having trouble accessing your PCP for an updated physical examination in time for the start of the season, you can also submit a supplementary Preparticipation Physical Evaluation Form in lieu of the written proof of physical documentation.  This documentation will be reviewed by the athletic trainer and/or the school nurse, with follow up consultation as necessary, before confirming authorization for participation in Mt. Ararat High School Athletic programs.
